Event Registration Form
What should an event registration form include?
An event registration form collects what you need to admit, badge, feed and follow up with each attendee, and nothing more. Most forms need the attendee's name, email, company and job title, the ticket or registration type, and a handful of questions that change what you do: sessions, dietary and accessibility needs, and consent. Paid events add payment, and business events often add questions that help with networking or lead qualification.
Every extra question is one more reason to abandon the form, so each one should have a use you can name. Below are the fields to include and why, a copy-ready form template, extra questions by event type, what belongs on the registration page around the form, and how to build an online event registration form.

Fields to include on a registration form
Use this as a starting point and delete what you will not use. Ask a question per attendee when the answer differs by person, such as dietary needs, and once per order when it does not, such as the billing address.

Extra questions by event type
Beyond the basics, add only the questions your type of event actually needs:
Conferences
Tracks, workshop choices with limited places, and whether the attendee is a member or a speaker.
Trade shows
Attendee type (buyer, exhibitor, press), industry, company size, products of interest and purchasing role, which exhibitors use to qualify leads.
Trade show registrationBusiness matchmaking events
What the attendee offers and what they are looking for, so meetings can be matched.
Business matchmakingCorporate and internal events
Department, office location, travel and hotel needs, and manager approval if your company requires it.
Galas and fundraisers
Guest names for each seat, meal choices, seating preferences and an optional donation.
Fundraising event softwareVirtual and hybrid events
Time zone, whether the attendee joins in person or online, and captioning needs.
Hybrid event platform
The event registration page around the form
The event registration page is where the form lives. Before anyone starts filling it in, the page should answer the questions that decide whether they register at all:
What, when and where
Event name, dates, times with the time zone, and the venue address or how to join online.
Who it is for and why
A sentence or two on the audience and what they will get, then the agenda and speakers.
Prices and what is included
Each ticket type, its price, its deadline and what it gives access to, including early-bird or group pricing.
Policies
Refunds and transfers, the code of conduct, and how attendee data is used.
One clear button
A single Register action near the top of the page, repeated at the end of a long page.
Works on a phone
Many people register from an email on their phone, so test the whole flow on a small screen, including payment.
Registration form best practices
These habits mean more people finish the form and the data you collect is cleaner:
Ask only what you will use
If nobody can say what an answer is for, cut the question.
Keep the required list short
Mark required fields clearly. Optional questions can wait for a later email or the event app.
Offer choices instead of free text
Dropdowns and checkboxes give you answers you can count; free text gives you ten spellings of the same thing.
Confirm straight away
Send a confirmation email with the date, venue, calendar details and the QR code for check-in.
Decide what happens when it fills up
Choose in advance whether a sold-out ticket closes or opens a waitlist.
RSVPs and waitlistsTest it end to end
Register yourself on a phone and a laptop, pay, and check that the confirmation and the data arrive as expected.

FAQ
Event Registration Form: common questions
What information should be on an event registration form?
Name, email, company, job title and ticket type, plus the questions that change what you do: sessions, dietary and accessibility needs, and consent to terms and recording. Paid events add payment.
How do I create an online event registration form?
Set up your ticket types in a registration tool, add your questions with required fields marked, design the page, connect payment if the event is paid, and test the whole flow on a phone before sharing the link.
What is the difference between a registration form and a registration page?
The form is the set of questions. The registration page is everything around it: the event details, agenda, prices, policies and the button that starts the form.
How many questions should a registration form have?
As few as you will use. The essentials fit on one screen; add a question only when the answer changes what you do, such as catering, session capacity or badge type.
Should I ask for dietary and accessibility needs at registration?
Yes, if food is served or the venue needs to make arrangements. Asking at registration gives the caterer and venue time to plan.
Can I put a registration form on my own website?
Yes. Most registration tools give you a link or embed code. Eventdex lets you embed the form inline, as a popup widget or popup text, or share a short URL.
